Follow the steps below to complete the Online Application.

Be sure to review our General Admissions criteria. New applicants are required to complete the online application.

Step 1

Register as a new applicant by accessing FCA Sycamore Online Application Portal. You will receive an email with a link to activate your new account and create a password. Please be sure to check your SPAM if you’ve not received a link after 15 minutes.

Step 2

Once you’ve received your new user ID and set your password, log into Sycamore to complete the online application. 

After you complete the application, you must pay the $95 registration fee before your application is processed.

Step 3

Submit the required documents below to complete the application process. All documents are accessible through Sycamore.

  • Health Statement Form (K – Grade 8)
  • Records Release Form (K – Grade 8)
  • Annual Physician’s Statement Form (18 months – 4 years only)
  • Reading Teacher Evaluation (K – Grade 8)
  • Math Teacher Evaluation (K – Grade 8)
  • Principal/Counselor Evaluation (K – Grade 8)
